Discovery and Design

The Discovery and Design (DDD) stage is where you will plan your ILO services together with Triniti Care or another service provider.

At this point, you will:

  • Identify your needs, goals, and preferences
  • Look at various housing and support options
  • Get input from family, friends, and/or carers, if desired
  • Develop a personalized service plan based on your individual lifestyle.


The purpose of DDD is to create a customized service plan for ILO.

Implementation and Ongoing Support

Once your plan is ready and approved, it’s time to bring your plan to life.
This includes:

  • Arranging your preferred living situation
  • Getting the right amount of support
  • Reviewing and changing services as your needs change

Disability living support NDIS is not just about living in one way. Your support services are designed to change and grow with you.
