We spend time to initially understand each of our client's individual situations along with their needs and personal goals. We work closely with each client support people with disability to enable them to feel respected, empowered and cared for. To best understand each client's needs and expectations, we develop individual Service Agreements and Support Plans to best support you.
The Service Agreement ensures there's a collective understanding of the support services to be provided. It also covers the participant's expectations, our responsibilities to you, and allow us to identify the risks you may be exposed to so these risks can be appropriately managed. Our Support Plans help to identify your needs and support requirements, your strengths and goals, and additional proactive measures which assist you in maintaining a healthy lifestyle.
