Our Services

Triniti Home Care’s NDIS services include Household tasks, Therapeutic Supports, Assistance with travel/transport arrangements, Home modification design and construction, Community nursing care for high needs, Interpreting and translation.

Assisting Personal Activities

Assisted personal activities involves supporting participants with their day-to-day activities. This includes assisting participant's with:

  • showering, bathing, dressing, oral hygiene, and grooming;
  • bladder and bowel management, and toileting;
  • eating and drinking;
  • attending appointments;
  • use of appliances and aids, communication and hearing devices;
  • being mobile at home ie, support moving on and off the toilet and in and out of bed; or
  • basic first aid administered due to the participant's disability, and application of splints.

Assisted personal activities can be required in a range of circumstances. This includes supporting participants who live alone, with family or other people; and assisting client's to undertake educational, recreational, social, or employment tasks, and when taking holidays.

Household Tasks

We help our clients with essential household services which they are not able to perform independently. Includes meal preparation, home and garden maintenance, cleaning and laundry.

Meal preparation services involve the development of participant meal plans where the following factors are taken into account:

  • allergies,
  • food/drink preferences,
  • religious food requirements,
  • eating and drinking supervision requirements,
  • meal times,
  • whether the participant can eat out,
  • whether the participant has a complex physical disability and requires positioning and turning of food to maintain airway safety and avoid choking risk and in pressure care.

Home and garden maintenance, cleaning and laundry are undertaken with a professional approach. We ensure these services are undertaken in a way which maintains the safety and wellbeing of our clients, and that helps them to feel proud of their home environment.

Home modification, design and construction

Home modifications are changes to the structure, layout or fittings of a participant’s home which provides our clients with safe home access and the ability to move around comfortably in areas they frequently use.

Home modifications may be undertaken due to:


  • the impact of a participant's disability, a participant or their carers may not reasonably be able to access and use frequently used rooms and spaces in the primary residence;
  • the participant's primary residence, in its current condition, may have significant and adverse impacts on the sustainability of current living and care arrangements; and
  • a suitably qualified Occupational Therapist has performed an assessment and recommended home modifications, considering all possible alternatives, including the use of equipment.

Assisting Travel/ Transport

We provide travel and transport services to people with disability. Assistance with travel and transport includes supports that enable participants to build capacity to independently travel, including through personal transport-related aids and equipment, or training to use public transport.

We work with our clients to ensure that they are able to use transport in a safe way by understand the risks and ensuring the required mitigation measures are implemented. This is achieved with public transport by assessing the participant's ability to understand and manage the relevant risks, and through our organisation's vehicle transportation services by ensuring that the specific aids and equipment are in place to safely transport our clients.

Community nursing care for high care needs

Our community nursing care for high care needs includes support and care for wounds, stomas, and respiratory issues. Individual participant care plans are developed for these services which involves an:

  • assessment of our client's past and present health conditions, and the associated treatments used,
  • the participant's support needs and the required frequency of this support,
  • hygiene measures,
  • monitoring a participant's stoma (where present),
  • enteral feeding requirements,
  • tube feeding requirements, and
  • additional medical issues that have developed as a result of the participant's initial health conditions,
  • risk factors which can lead to further complications,
  • approaches applied to prevent or reduce health issues,
  • ongoing health assessments,
  • treatments,
  • care and maintenance of medical and other equipment, and
  • emergency medical support.

Allied Health Service

Allied health providers deliver therapeutic support to participants and are one of the largest groups of registered providers.

Our allied health professionals will work with you to provide the best care possible so that you can live an independent and active life. Our allied health services include physiotherapy, occupational therapy, and speech therapy.

Physiotherapy

Our physiotherapists are involved in the assessment, diagnosis, planning and management of patient care. They are experts in the structure of the human body and its movement and they work with participants of all ages to treat a broad range of health conditions.

Occupational Therapy

Our occupational therapists focus on promoting health and wellbeing by enabling participants to participate in the everyday occupations of life, such as self-care activities including showering, dressing, preparing food; productive activities including work, volunteering, education and caring for other people; and recreational/social activities, this includes taking on a hobby, and joining a community group or a friendship group. Our occupational therapists play a vital role in assisting people with a disability to identify and implement strategies which support their involvement in occupations.

Speech Therapy

Our speech pathologists diagnose and provide treatment for communication disorders. These communication disorders include difficulties in understanding language, listening, speaking, writing, reading, stuttering, and social skills. The participants we support may include people who have difficulty communicating as a result of stroke, learning disabilities, developmental delays, dementia and hearing loss, intellectual disability, brain injuries, cerebral palsy, along with other issues which can impact language and speech. In addition to the above, our speech pathologists can also assist people who experience complications when swallowing food or when drinking.

Interpreting and traslation needs

We support our clients in making contact with and utilise NDIS translation and interpretation services. These services assist our staff to work with people from all different nationalities, and enables people with limited English to engage with a wider range of people and community groups.

Group/Centre Based Activities

Triniti home care has Centre-based Activities and Groups to promote socialisation. These Centre-based Activities are facilitated by activities staff and support workers and have self-directed skill-building activities.

Daily Tasks / Shared Living

Triniti Home Care assists with and supervises tasks of daily life in a shared living environment. With a focus on developing the skills of each induvial to live as autonomously as possible. We provide respite and permanent accommodation.

We understand that living independently can sometimes feel impossible, but we can make it possible. Our expert staff will support you to live with others while ensuring you can fulfil your responsibilities around the home, assisting you with your move into your new home or assisting you with daily Tasks, which include activities of daily living, preparing meals, community participation, and enjoying your life without having to depend on others.

Participate Community

Triniti Home Care enables social and community participation for participants. The participants participate in social, community, and recreational activities in a centre or community setting. This support category assists an NDIS participant in activities such as tuition, coaching, and skill building, such as art classes, sports coaching, and activities that build skills and independence.

Community access can be as one-off appointments or as ongoing support to assist participants in activities of their choice in the surrounding community.

Plan Management

Triniti Home care's Plan Management understands and cares about you, your goals, and your aspirations. We know that navigating through NDIS can be a difficult and time-consuming process. As your plan manager, we are here to take the stress out of managing your NDIS plan and budget so you can make the most out of it! This includes submitting claims to the NDIS portal, paying invoices, and assisting with any NDIS-related issues.

Triniti Home Care services serve various people seeking support and accommodation across Sydney, Canberra, and Melbourne. Triniti Home Care operates a range of supported accommodation homes for people with disability. Our disability housing options include individual group homes and independent housing facilities in Sydney and Melbourne.

Support Coordination

Support Coordination helps you navigate and achieve your NDIS plan and goals by connecting you to community support. Triniti Home Care enables you to maximize your NDIS plan by considering your specific needs and providing support around any challenges you may face. Our Support Coordination team understands your NDIS, gets the proper support in place, and assists in building your capacity for independence. We'll ensure you get service tailored to your needs and goal and work closely with you to unpack your plan and help you understand how to use it. We will guide you in selecting local supports that best meet your needs and achieve your goals and Negotiate service agreements between you and your providers. Finally, we will review your support services to ensure they work well.
