Our community nursing service for people with high care needs includes support and care for wounds, stomas, and respiratory issues. Individual participant care plans are developed for these services, which include:
Wound care
Assessment of our client's past and present health conditions, and the associated treatments used,
The participant's assistance requirements, the required frequency of this assistance,
Hygiene measures,
Monitoring a participant's stoma (where present),
Enteral feeding requirements,
Tube feeding requirements, and
Additional medical issues that have developed as a result of the participant's initial health conditions,
Risk factors which can lead to further complications,
Approaches applied to prevent or reduce health issues,
Ongoing health assessments,
Treatments,
Maintenance and upkeep of medical and other equipment
